Around one in six Australian couples experience difficulty conceiving. Most fertility investigations focus on sperm quality or hormone levels, but the immune system can also play a role. Antisperm antibodies are immune proteins that attach to sperm and interfere with how they move and function. They're more common after vasectomy, testicular injury, surgery or infection, and can develop in either partner - which is why this test is suitable for men and women.
Because it's done on a blood sample, there's no need to provide a semen sample. In the laboratory, your blood sample is incubated with donor sperm and the proportion of sperm that become antibody-coated is measured. The result combines two antibody types, IgG and IgA, and is reported as a percentage. Reference limits follow the WHO Laboratory Manual for the Examination and Processing of Human Semen.
If you're a man being investigated for fertility, your practitioner may recommend this alongside a semen analysis, as the two look at different things.
